Fields: country and state of residence, case URL, relationship to record, sealed-or-expunged yes/no. Selecting 'I am a party' expands the form to demand first and last name, full street address, city, state, postal code, email twice and phone twice. The reason field is a four-option radio and NONE of the options fit an ordinary user; only the catch-all 'I don't believe public records should be published online' is truthful for most people, and it is the weakest. Submit carries a penalty-of-perjury certification, so it can never be auto-submitted. After submit: a verification email with a 14 day window, and the link leads to a SECOND form to pick the party. Observed outcome was NOT approval but a 6 month TEMPORARY redaction, reverting unless a court sealing order arrives, with a 30 day appeal window. Appeals are refused without substantive new documentation. Set expectations accordingly in the UI.

For self-authored pages the correct route is delete or restrict at the platform first, then google_outdated. Route users accordingly instead of burning a request. The form also requires a screenshot upload and two of its fields (search results page URLs, search terms) reject programmatic input, so the final step must run in the customer's own browser.

A page already dropped from the index returns 'Denied: Page not indexed', which is a SUCCESS state and the UI must present it as one rather than as a failure.
